1 When you're young

Don't die early

Get the HPV vaccine for women, the earlier the better

In data on 1,670,000 (167ไธ‡) women in Sweden, those vaccinated against HPV before age 17 had only about one-eighth the chance of later getting invasive cervical cancer compared with those not vaccinated. Those vaccinated between 17 and 30 had about half. So the earlier you get it, the more it is worth.

Don't waste your energy

For period pain, take a painkiller such as ibuprofen rather than toughing it out; if the pain keeps you from work or school, or three months of medicine doesn't control it, see a gynecologist

For period pain, taking a drug such as ibuprofen raises the share of people whose pain clearly eases from under two in ten to about half. It works better than acetaminophen, with somewhat more side effects. If the pain keeps you from work or school, hurts deep inside during sex, or isn't controlled after three months of medicine, see a gynecologist to be checked for endometriosis. In a study across 10 countries, this disease went on average 6.7 years from the first symptoms to diagnosis.

Women over 30 should get cervical cancer screening, with an HPV test first

In the rural India trial, women who had just one HPV test in their lives were about half as likely to die of cervical cancer, and about half as likely to be found with advanced cervical cancer. At one or two hundred CNY a test, the U.S. recommendation is to test every 5 years.

2 After 40

Don't die early

Women should start breast cancer screening at 40, with a mammogram every 2 years

A mammogram every 2 years for women aged 40 to 74 is explicitly recommended in the official U.S. screening advice. For women 75 and older, and for adding ultrasound or MRI for dense breasts, the official conclusion is that the evidence is not enough. When a checkup package charges extra for those, you can skip them.

Don't die slowly

If hot flashes and night sweats around menopause are affecting your life, see a gynecologist about hormone therapy; don't just put up with it, don't buy the drugs yourself, and don't use it to prevent heart disease

For hot flashes and night sweats around menopause, hormone therapy cuts the number of hot flashes by about 75% compared with a placebo. The cost is a little more breast cancer, blood clots and stroke, about 8 more cases of each per 10,000 (1ไธ‡) women per year. For people who start at age 50 to 59, overall mortality does not rise. It cannot be used to prevent heart disease or dementia.

3 After 65

Don't die early

Women over 65 should get one dual-energy X-ray bone density scan, and after menopause, women with risk factors for osteoporosis shouldn't wait until 65

Women over 65, and women after menopause who have risk factors for osteoporosis, should have their bone density checked once. Checking risk first and then treating the high-risk people reduces hip fractures by about 17%. In numbers, for every 1000 people screened, about 5 fewer have a hip fracture. No change in total death rate was seen.

Don't die early

If osteoporosis is found, or you have broken a bone from a light fall, ask a doctor to prescribe an anti-osteoporosis drug and keep using it; calcium supplements are not a substitute

For postmenopausal women who have been diagnosed with osteoporosis or have broken a bone, taking alendronate reduces hip fractures by about half and symptomatic spine fractures by more than half. For older adults who start a once-a-year infusion after hip fracture surgery, new fractures over 2 years are about 35% fewer. Calcium alone cannot stand in for these drugs.
